Genetic precision medicine in obesity and diabetes

Abstract

Precision medicine promises to customize treatments based on genetic data for obesity and diabetes, facing challenges such as lack of adherence and side effects. The aim of this study was to understand the applicability of using genetic variants in precision medicine for the treatment of obesity and diabetes. A systematic review was conducted following the PRISMA 2009 guidelines, searching for scientific articles published from 2017 to 2023 in PubMed, SciELO, and Google Scholar using the descriptors "precision medicine" AND "obesity" and "precision medicine" AND "diabetes". We found 10 studies on obesity and 20 on diabetes. The studies identified genes and their respective proteins such as LEP (Leptin), LEPR (Leptin Receptor), MC4R (melanocortin-4 receptor), and POMC (proopiomelanocortin) as potential genetic markers for obesity, and genes and their respective proteins like HNF4A (Hepatocyte Nuclear Factor 4 Alpha), GCK (glucokinase), HNF1A (HNF1 Homeobox A), and ABCC8 (ATP-binding cassette subfamily C member 8) as influential for diabetes. Promising examples include weight loss in patients with common obesity and POMC deficiency, and glycemic stabilization in patients with mutations in genes causing MODY diabetes, responsive to sulfonylureas. Although the study identified genes with potential to guide future treatments, further research is still needed for the development of effective therapeutic interventions in precision medicine.
